Please complete both the Registration form and the Medical Release form in their entirety. You do not have to actually provide your physician’s signature; we are only looking for contact information in case of an emergency. Please mail the completed registration form and medical release form as well as your payment ($155) to:

Uniform Information: You must purchase your uniform package by April 17th (second sign-up date) in order to receive the bulk order price. People who still need a uniform package after April 17th will be asked to contact Wayne Fields at Sports Machine (9033 Colerain Avenue, 513-741-8333) directly.
